Explained in plain language

What is this test?

Direct bilirubin is the water-soluble form after processing by the liver. It helps clarify whether raised total bilirubin may relate to liver-cell processing or bile flow.

Why might it be ordered?

  • To investigate jaundice or an abnormal total bilirubin.
  • To assess possible liver or bile-duct problems with other liver tests.

What sample is needed?

A blood sample is required. The laboratory chooses the correct tube and handles the sample for this test.

How should you prepare?

Ask the laboratory whether you need to fast or attend at a particular time. Do not stop or change medicines or supplements unless your clinician tells you to.

How should you understand the result?

Direct bilirubin is interpreted with total bilirubin, AST, ALT, GGT, alkaline phosphatase and symptoms. It does not identify the cause on its own.
